Before scheduling your appointment, please review and prepare the required materials listed below. Without correct documentation and fees, we will not be able to process your application, and you will need to schedule a new appointment.
Note: All Department of State application fees must be paid via personal check, cashier's check or money order only. All execution fees must be paid via debit or credit card.
What to Bring With You

Completed Passport Application (DS-11)
You must fill out a DS-11 form if:
- You are applying for the first time
- Your previous passport was a 5-year passport
- You do not meet the requirements to renew your passport
You can print out the form and fill it in with black ink or use the Travel Department's online form filler. For either option, please bring the complete printed form to your appointment. Do not sign your application. Applications must be signed in person when directed by a passport agent.

Valid Passport Photo
A new passport photo is required for each application.
Passport photo services are available at our offices. The fee for photo services must be paid via credit/debit card as part of the execution fee.
You can also bring your own passport photo to use with your passport application. All photo submissions (adult and children) must meet the following U.S. Department of State requirements:
- Sized 2"x2" with the head centered in the frame
- Taken in the last 6 months
- Clear, in color and properly exposed with no shadows
- Background is uniform, plain white and free of shadows
- Face is unobstructed:
- Hair has been adjusted so the entire face is visible
- No glasses (even prescription glasses should be removed)
- No hats or head coverings, except for religious or medical purposes
- Applicant is directly facing the camera with a neutral expression

Proof of Citizenship
Applicants must bring both the original and a copy (front and back) of their Proof of United States Citizenship.
Note: Photocopy services are available on-site for $1 per copy. The fee for photocopy services must be paid via credit/debit card as part of the execution fee.
Proof of Citizenship: Born IN the United States
Bring one of the following:
- Fully-valid, undamaged U.S. passport (can be expired)
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county or state of birth
- Lists applicant's full name, date of birth and place of birth
- Lists parent(s)' full names
- Has the signature of the city, county or state registrar
- Has the date filed with registrar's office (must be within one year of birth)
- Has the seal of issuing authority
Proof of Citizenship: Born OUTSIDE the United States
Bring one of the following:
- Fully valid, undamaged U.S passport (can be expired)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship

Identity Verification
Applicants must bring both the original and a copy (front and back) of their Identity Verification. Digital IDs are not accepted.
Note: Photocopy services are available on-site for $1 per copy. The fee for photocopy services must be paid via credit/debit card as part of the execution fee.
Primary Identity Verification
Bring one of the following:
- Valid, undamaged U.S. passport book or passport card
- In-state, fully valid driver’s license
- Certificate of Naturalization
- Certificate of Citizenship
- U.S. military or military dependent ID
- Current (valid) foreign passport
- Matricula Consular (Mexican Consular ID)
- U.S. Permanent Resident Card (Green Card)
- Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
- Enhanced Tribal Cards and Native American tribal photo IDs
If you cannot present one of the Primary Identity Verification methods, you must bring two Secondary Identity Verification methods.
Secondary Identity Verification
Bring two of the following (if you do not have a Primary Identity Verification:
- Out-of-state driver's license or enhanced driver's license with photo
- Expired driver's license
- Learner's or temporary driver's permit (without a photo)
- In-state, fully valid non-driver ID (without a photo)
- Out-of-state, non-driver ID
- Temporary driver's license (without a photo)
- Social Security card
- Voter registration card
- Employee work ID
- Student ID
- School yearbook with an identifiable photograph
- Selective Service (draft) card
- Medicare or other health card
- Form DS-71 for an Identifying Witness

Payment Method
Execution fees to UCR can be combined, but they must be paid separately from fees to the U.S. Department of State. Please note: Department of State fees must be paid by check or money order only.
